Plan Enrollment, Eligibility & Forms

If you are an active CU Medicine employee, this page includes important information about plan enrollment, deadlines and elgibility. 

How to Enroll

STEP 1: Read this page to learn about enrollment deadlines benefit plans, eligibility and educational resources. This site contains all of the necessary forms you will need to enroll or change your benefits.

 

STEP 2: Ask for help. Human Resources is available to answer your questions, direct you to other resources, and help you make informed decisions about your benefit options.

 

STEP 3: Enroll in Benefits. Enroll via the Dayforce website.  Mail, fax or hand deliver  any supporting documents to Human Resources.

 

STEP 4: Confirm your elections. Once your enrollment is processed, verify benefit enrollment via the Dayforce website.  Review your pay statement to verify your pay deductions for your intended benefits elections. Contact HR immediately if you have questions or concerns.

 

Enrollment Opportunities and Deadlines 

There are three occurrences during a plan year when you may enroll or make changes to your benefit plans

New Employee/Newly Benefits Eligible Employee

As a new employee or a newly benefits eligible employee, you must enroll within 31 days of your date of eligibility.

Life Events

Certain changes in your or a dependent's employment, or other life changes for you or a dependent may be considered qualified family status changes or other life events. These may enable limited changes in your benefit elections within a restricted period of time. Human Resources must be notified and in receipt of proper supporting documentation within 31 days of the date of the event. For more information, visit our Life Events page.

Open Enrollment

The annual open enrollment period is held in the spring. Open enrollment deadlines and other details are posted on the CU Medicine website as soon as the information becomes available.


Benefits Eligibility

Individuals must be paid on CU Medicine's monthly pay cycle and be appointed to a 50 percent or greater benefits-eligible position in order to be eligible for CU Medicine's group benefits plans. Eligibility date is the first of the month following your start date. In the event you start on the first of the month, your eligibility date will be the same as your start date.

COBRA

COBRA – The Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) enables departing employee to keep some health insurance benefits for a limited amount of time.
