Primary care is founded on a longitudinal, trusted relationship between patients and their primary care clinicians and associated care teams.

The essential elements of primary care include: serving as a patient’s initial point of contact to the healthcare system, providing person or family centered comprehensive, continuous, and coordinated care over time, with engagement and orientation to the overall community. . For patients, a primary care office is where you go to a Family Medicine or Internal Medicine physician for “anything and everything that comes up” and the place that you circle back to for updates, care coordination, and follow up. 

At the heart of primary care is your dedicated Primary Care Provider (PCP). Your PCP is your trusted healthcare partner, offering personalized and continuous care to address a spectrum of health concerns. Primary care, as an entry way to our health system, plays an important role in connecting people to emergency department, specialty, and inpatient care, as well as mental health and substance use disorder and treatment. 

Our Primary Care services bring together the expertise of both Family Medicine and Internal Medicine providers to ensure that patients of all ages receive the highest standard of care. 

Family Medicine providers specialize in caring for individuals and families, from newborns to seniors, fostering long-lasting relationships and providing a continuum of care. Our Family Medicine physicians work within Family Medicine teams that include Physician Assistants, Family Nurse Practitioners, and Psychologists, who excel at caring for individuals and families.  Family Medicine physicians are trained to care for patients of all ages, and to manage the majority of issues and medical conditions that patients seek initial care for. Within our system, Family Medicine physicians provide care for general medical conditions such as diabetes, hypertension, arthritis, asthma, and other common illnesses. They provide comprehensive care for women; obstetric care including prenatal care and labor and delivery; care for children, care for older adults;  and care for patients with depression, anxiety, and other mental health issues. Although the care at each of our practices may vary based on location and the surrounding community,  all of our practices  have behavioral health, social work, and care managers available for our patients. In many cases, Family Medicine physicians will provide complex, comprehensive care for patients struggling with more complicated clinical problems – often in partnership with General Internal Medicine or other Medicine or Surgical subspecialties.

Internal medicine is a branch of medicine that deals with the diagnosis, treatment, and care of adult patients, typically those aged 18 and older. Physicians specializing in internal medicine, also known as internists or doctors of internal medicine, undergo extensive training to become experts in managing complex medical conditions affecting adults. They are skilled in diagnosing challenging medical issues, managing severe long-term illnesses, and providing care for patients with multiple chronic conditions.

Internists are known for their ability to approach patient care comprehensively, considering not only the presenting symptoms but also the underlying causes and potential interactions among various health issues. They are trained to handle a wide range of medical problems, from common ailments to rare diseases, and they often serve as primary care physicians for adults.

One of the key aspects of internal medicine is its focus on preventive care. Internists work to promote wellness and disease prevention through screenings, vaccinations, lifestyle counseling, and other interventions aimed at reducing the risk of developing chronic illnesses such as diabetes, heart disease, and certain cancers.
